Intake & Policies

When you arrive for your first appointment, you will be required to fill out a brief intake form and sign the policies page.

If you'd like to save time for when you arrive, you may download, print, and sign. Please make sure you sign both the intake form and policy page and bring it with you on your first appointment. Otherwise, you may fill both out at my office.
